Finding the Information You Need

MN Community Measurement greatly appreciates the health care providers who submit data on quality, patient outcomes and patient experience to us for the purpose of measurement and reporting. Your contribution is vital to our larger community, as well as MNCM’s ability to accelerate the improvement of health by publicly reporting health care information.

To do so, it’s important that we support to your success as well. MNCM provides medical group-specific reports; comparable results; and tools that your medical group or clinic can use to inform quality improvement initiatives. These resources are available in varied levels of detail in three locations: our public reporting website,; our corporate website,; and our secure MNCM Data Portal.

Our consumer website,, has current-year results on all of MNCM’s publicly reported provider performance measures. These include cost and quality ratings on more than 1,600 clinics; 500 medical groups; and nearly 140 hospitals in Minnesota and neighboring communities. The comparable results can be sorted by HealthScore, rate, geography and alphabetical order.

The site focuses on providing information patients need to make smart care choices in formats that have been proven to resonate with and be understandable for most patients. The data displays are more visual and less numeric, as well as provide less detail than a health care professional may want. The resources and educational tools on the site are for patient use as well.

How can this help providers? is an excellent resource to discuss with your patients when they’re looking for procedure cost information; or ratings on an outside clinic that performs care that yours does not. Additionally, the patient-friendly resources on diabetes, asthma and more can be helpful as patient education and condition management tools.

The Health Care Quality Report and Total Cost of Care Report offer more detailed provider performance results and analysis than what is available on For instance, the Quality Report includes regional breakdowns, confidence intervals and numerator / denominator counts; the Cost Report includes regional analyses, Total Cost Index ratios, breakdown and distribution of risk, and breakdown of cost by type of care.

The Heath Equity of Care Report and Health Care Disparities Report provide measure results segmented by race, Hispanic ethnicity, preferred language, country of origin and insurance product. Segmented results are not available on

How can this help providers?

All of MNCM’s provider training and education tools are available on, as well as resources for patient education and engagement. Additionally, this is the place for health care leaders, quality improvement professionals and others to find detailed and/or segmented provider performance results that are comparable.

MNCM Data Portal

The MNCM Data Portal is used primarily to submit data for performance measures to and communicate with MNCM through a secure, HIPAA-complaint channel. In addition, it’s an area where medical groups can access more granular and detailed data on their specific group than what is publicly reported. To access the Data Portal, and the information below, you must be assigned login credentials by your medical group or contact the person in your medical group who is assigned.

Available resources that are specific to your medical group include:

  • Detailed reports for clinical quality measures, including separate components within the Optimal Diabetes Care and Optimal Vascular Care composite measures; your group’s previous results for trending purposes; and your group’s results segmented by insurance product.

  • Detailed reports for historical Patient Experience of Care survey results, including details on which survey questions that roll up into each of the survey’s four domains.

  • Charts of your group’s performance on certain clinical measures segmented by race, Hispanic ethnicity, preferred language and country of origin (only available to groups following best practice).

How can this help providers?

Due to its security and privacy, the most detailed and specific data available from MNCM about your medical group can be found in the MNCM Data Portal. This can provide the most actionable information for setting internal quality improvement benchmarks and goals. In addition, medical groups that submitted data but were not eligible for public reporting due to having fewer-than-necessary eligible patients can view their results. This gives these groups a sense of their performance despite not meeting the level of validity necessary for public reporting.
